7x7" two-sided glossy paper sleeve only for single released 1/1964 by Vee-Jay records. B-side was "From Me To You." Same design on each side including great band photo. Back has inked name above John and slight fade to band name. Lt. aging/handling wear but no splits. Front displays nicely. VG+.
