6.5" tall blue lid with velvet lined gold bottom box which has large label "750 Club". Badge is 4.5" tall for use at Los Angeles convention and with a "750 Club" metal plate on the top hanger. Various back markings by maker Bastian Bros., Rochester. Box VF, Badge NM. Only example we've seen so designated.
