Premium consists of a large die-cut stiff cardboard "Totem Head" designed to fit over head and includes 2 different "faces." Other parts include cardboard sheet with punch-out ear and nose pieces plus original business-size envelope with printing noting the enclosed "Magic Forehead Vision." This is a thin silver plastic sheet that attaches over the eye opening. N. Mint.
