6.5x8.25" double-sided application dated Oct. 5, 1938. Application is for Los Angeles local chapter of the Performer's Union that would later become The American Federation Of Television And Radio Artists. Fonda has signed and filled out application in blue ink filling in his address and phone number. Two staple holes at right-hand margin. .25" tear on left-hand margin. Small 1/8" snag in upper left-hand corner. Folded as mailed. Exc. Comes with Hake's COA. See Bette Davis application #1124.
